 You may apply on-line and submit payment via credit card using our on-line secure application from below. If you prefer, you may download and print the Adobe Acrobat version of the application form (requires the free Adobe Acrobat Reader), fill out and either fax it with your credit card informationm or mail it with a check or money order for the registration fee. | |||||||||||||||||||||||||||||||||

Payment Method:
Refund Policy: Students may only receive a refund if he/she notifies Peridance Capezio Center at least one week prior to the beginning of their Workshop Series. If refund is given a 15% registration and processing fee will be deducted from the refund amount. Refund for payments made by Credit Card will bear an additional deduction of 3.5%(to cover credit card fees). If peridance is notified less than a week before the Workshop Series begins, students may be offered class credit only. Peridance reserves the right to cancel a workshop if enrollment is insufficient. In such case student will receive a full refund or class credit.
